The application forms must be turned in with the following documents:
 
Original Certified Birth Certificate (A copy will be made in the school office.)
Original Social Security Card (A copy will be made in the school office.)
Immunization Records
Baptismal/First Communion Certificate when applicable
Copy of Most Recent Report Card (1^st– 8^th)
Copy of Standardize Testing when applicable
 
Students applying for grades K-8^th will be required to take a placement test.
